A flat bronze gravestone plaque embedded in the ground, surrounded by green grass with some white lichen or moss spots on the stone and edges. The inscription reads: 'GONE BUT NOT FORGOTTEN' at the top. Below it, 'ANNA MARY WELLS' with birth year '1893' and death year '1976' separated by a small Christian cross symbol. Underneath, 'BERTHA E. COLLINS' with birth year '1887' and death year '1977' also separated by a small Christian cross symbol. The plaque has a weathered bronze appearance with verdigris patina. No additional decorations, flowers, flags, or mementos are visible. A small red leaf is present near the top right edge.
